Before you complete the Enrollment Application below, please carefully read and sign the Application Agreement. We've provided two ways for you to sign the application. You can download it, or you can sign it electronically. Downloading will require you to scan and email the form back to us or mail it to us. Instructions if downloading the Application Agreement:

Once you download the agreement, print it and fill in the required information. To return the form to us, you have two options:

Waiting Period: Please allow two weeks from the day you pay your enrollment fee to receive an acceptance or reconsideration.
